Any report made pursuant to this subdivision (b)(1) shall be accompanied by the following information, to the extent that such information is known to the person making the report: (A) The name, mailing address, telephone number, and email address of the minor’s parent or legal guardian and (B) The name, mailing address, telephone number, and email address of the person who tattooed the minor. (b) (1) Any parent, legal guardian, teacher or medical provider or school resource officer for a minor under eighteen (18) years of age, who discovers that a minor has been tattooed is encouraged to report such discovery to the department within three (3) weekdays of making such discovery. written: includes printing, typewriting, engraving, lithography, and any other mode of representing words and letters.United States: includes the District of Columbia and the several territories of the United States.Tattoo: means any method of placing designs, letters, figures, symbols, cosmetics or any other marks under the skin of a person with ink or color by the aid of needles or instruments.Person: includes a corporation, firm, company or association.Minor: means any person who has not attained eighteen (18) years of age.Guardian: A person legally empowered and charged with the duty of taking care of and managing the property of another person who because of age, intellect, or health, is incapable of managing his (her) own affairs.Discovery: Lawyers' examination, before trial, of facts and documents in possession of the opponents to help the lawyers prepare for trial.Department: means the department of health.
